For the most part, urge incontinence is
treated with medications and behavioral therapy,
both of which are highly effective. In the event
incontinence problems are severe - and you have
not responded to other treatments - your doctor
may suggest a procedure known as "sacral
neuromodulation" - a type of nerve stimulation
that is linked to urine control.
Normally, there is continuing communication between your bladder and nerves in your spinal column, which send impulses that tell you when to urinate and when to "hold shut". If the signal isn't strong enough, your bladder doesn't get the message - so it remains open too long, or opens too frequently.
In this procedure, your doctor will implant a thin electrical wire under the skin in your lower back. The wire runs under your skin until it is connected to a tiny battery operated electrical device (about the size of a stop watch) that is also implanted under your skin near the back of your abdomen.
You use a hand-held device to control a mild electric current that runs from the box to the sacral nerve in your lower back. This simulates the normal nerve message that should be relayed to your bladder to help control urination. The effect is permanent and requires only a small maintenance operation every 10 years to replace the battery.
In one study, 45% of women with urge incontinence remained completely dry, while 34% cut leakage events by one-half or more. Almost two-thirds of the patients were able to reduce their trips to the bathroom by more than half.
However, one-third of the patients also needed additional surgery for complications, including pain and infection. Again, this is considered a dramatic procedure and is used only as a last resort, for severe cases of urge incontinence.
